Job Summary
Provide home and community-based services to children with emotional and mental health challenges within a Family-driven Care framework; travel locally to work with children and families in their communities, delivering core rehabilitative services, including non-licensed counseling and support for youth and family, skill-building, peer support, caregiver support and advocacy, pre-vocational services, and respite. Collaborate with families to identify goals, implement Treatment Plans, coordinate scheduling and provision of services, participate in treatment meetings and trainings, and complete documentation in support of youth and family outcomes.
Required Qualifications
- Bachelor's degree in a related field and at least two years of related experience OR Master's degree in Social Work, counseling, psych, or a related human service and at least one year of related experience
- Valid NYS Driver's License required
