Job Summary
Provide home and community-based services to children with emotional and mental health challenges, traveling locally to work with children and families in their communities; collaborate with families to set and implement goals within a Family-driven Care framework, develop and carry out treatment plans, coordinate scheduling and provision of services with the child and caregiver, participate in treatment meetings and trainings, and complete all required documentation; roles include non-licensed counseling, advocacy, skill-building, caregiver support, and respite within an on-site community setting.
Required Qualifications
- Bachelor's degree in a related field and at least two years of related experience OR Master’s degree in Social Work, counseling, psych, or a related human service and at least one year of related experience
- Valid NYS Driver's License required
