Job Summary
Conduct direct patient care services including drug regimen review, disease state management, medication counseling, adherence monitoring, discharge medication reconciliation, prescription dispensing, and immunization administration. Perform all pharmacy operations in accordance with accredited policies, Board of Pharmacy, and State and Federal requirements while maintaining patient record documentation. Manage inventory functions including 340B program integrity, controlled substance ordering, receiving, and dispensing. Support orientation, training, and development of community pharmacy staff. Ensure compliance with Baptist Health policies, REMS, third-party payers, and state/federal regulations. Coordinate safe transitions of care from the hospital. Requires Kentucky Board of Pharmacy license, Bachelor's or Doctorate in Pharmacy, BLS certification, and ability to travel to Baptist Health Hardin, Hardin Pavilion, and Bardstown.
Required Qualifications
- Kentucky Board of Pharmacy license in good standing
- Bachelor's Degree in Pharmacy or Doctorate of Pharmacy
- Current Basic Life Support (BLS) certification
- Valid driver's license
- Availability to travel to other community pharmacies within KY as needed (Baptist Health Hardin, Hardin Pavilion and Bardstown)
- Must obtain Immunization certification within 6 months
- Post offer, pre-employment drug testing
