Job Summary
Connect older adults to healthcare services, financial assistance, and community resources like housing, transportation, and food support. Coordinate care by linking participants, families, and providers while advocating for needs and educating families on available benefits. Maintain participant documentation, provide emotional support during challenging situations, and collaborate with nurses, social workers, and therapists to ensure comprehensive care. This role helps seniors maintain independence and avoid nursing home placement through personalized community-based support.
Required Qualifications
- High school diploma or equivalent
- Associate or bachelor's degree in human services, social work, healthcare, or a related field
- Experience working with older adults, healthcare, social services, case management, or community outreach
- Knowledge of community resources, Medicaid, Medicare, and public assistance programs
- Strong communication and interpersonal skills
- Ability to coordinate services and work collaboratively with an interdisciplinary healthcare team
- Organizational and documentation skills for maintaining participant records
- Valid driver's license and reliable transportation
Desired Qualifications
- Bachelor's degree in social work, psychology, human services, gerontology, or a related field
- Previous experience in a PACE program, senior services, behavioral health, or healthcare setting
- Experience assisting clients with benefits applications and care coordination
- Bilingual communication skills
- Familiarity with electronic medical records or healthcare documentation systems
- Crisis intervention or advocacy experience
- Knowledge of aging-related programs and long-term care services
- Strong networking skills and established connections with local community agencies
