Job Summary
Conduct telephone screenings and follow-up calls to assess consumer needs and eligibility for in-home or community-based programs. Provide information counseling and referrals by phone to help clients navigate services for long-term care, transportation, nutrition, and other needs. Act as a client advocate by assisting consumers with appropriate resources while informing them about available programs. Document consumer information collected from interactions within appropriate systems and monitor grant-funded program performance for compliance. Keep informed on aging and disability issues by gathering information about local service providers and state/federal investments. Work onsite on core business days with optional flexibility at the Manager's discretion.
Required Qualifications
- Bachelor's degree in social work or related field
- Two years of relevant experience
- Excellent communication skills
- Experience conducting phone interviews or counseling
- Proficiency with MS Office
- Ability to work both independently and within a team setting
- Proficiency in Microsoft Office
- Proficiency in Adobe Acrobat
- Onsite work on core business days
