Job Summary
The Community Resource Consultant (RN/LSW) evaluates individual or consumer needs, providing information on available options. Responsibilities include conducting face-to-face assessments, documenting information accurately, and advocating for clients. The role requires collaboration with caregivers and service providers, knowledge of community resources for older adults, and adherence to agency policies. Candidates must hold a relevant license and have two years of experience in applicable fields, along with excellent communication and assessment skills.
Required Qualifications
- Current licensure as a social worker by the State of Ohio Counselor and Social Work Board, or as a Registered Nurse with the Ohio Board of Nursing.
- Two years of experience in home care, geriatrics, home health care, medical social work, or other approved substitute experience by the Ohio Department of Aging.
- Excellent verbal and written interviewing and assessment skills.
- Ability to record information accurately and meet program timeframes.

- Knowledge of major chronic illnesses and disease processes
- Knowledge of local resources for older adults
- Excellent interpersonal and communication skills
- Intermediate computer skills including Microsoft Office applications
