Job Summary
The Community Resource Center (CRC) Community Health Coordinator II focuses on promoting members’ well-being by engaging with and navigating access to health services for attendees at assigned CRCs. Responsibilities include culturally mediated orientation and health education, system navigation, coordinating transportation, coaching through motivational interviewing, advocacy for individuals and communities, and building capacity through training and community outreach. The role emphasizes engaging with both members and non-members visiting CRCs, addressing barriers to care, guiding individuals to resources, and supporting chronic disease prevention and health literacy in a culturally and linguistically appropriate manner.
Required Qualifications
- High School Diploma or High School Equivalency Certificate
- At least 1 year of experience as a community health worker, health navigator, peer support worker, outreach worker, or promotora or working in a health care, social work or community-based setting.
- Knowledge of community resources in area of residence.
- Comfortable working with diverse populations.
- Excellent verbal and written communication skills.
- Able to work flexible job hours.
- Detail oriented, organized and possess time management skills.
- Basic computer skills.
