Community Relations Liaison

Job Summary
Develop and maintain strong relationships with hospitals, physicians, discharge planners, case managers, assisted living communities, and other referral sources; represent the facility at networking events, healthcare meetings, and community outreach activities to promote services and increase referrals; coordinate the admissions process from referral through resident arrival; verify insurance coverage and eligibility, including Medicare, Medicaid, and managed care plans; collaborate with discharge planners, families, and interdisciplinary team members to ensure smooth and timely admissions; conduct facility tours for prospective residents and families; serve as a resource for residents, families, and referral partners by providing information regarding services, programs, and admission requirements; ensure all admission documentation is complete, accurate, and compliant with regulatory requirements; track referral activity, admissions data, and census trends while maintaining accurate records; participate in census development initiatives and implement strategies to increase occupancy; provide a positive, professional, and supportive experience throughout the admission and transition process.
Required Qualifications
- High school diploma or equivalent required
- Associate's or Bachelor's degree in Healthcare Administration, Marketing, Business, Communications, or related field preferred
- Experience in healthcare admissions, business development, community relations, marketing, or skilled nursing preferred
- Knowledge of Medicare, Medicaid, managed care, and insurance verification processes preferred
- Strong relationship-building, networking, and customer service skills
- Excellent verbal and written communication abilities
- Highly organized with strong attention to detail
- Proficiency with Microsoft Office and electronic healthcare systems
