Job Summary
Provide in-home clinical nursing care for older Australians, including comprehensive clinical assessments, wound care, medication management, diabetes monitoring and education; develop, review and update clinical documentation and care plans; identify, assess and escalate clinical risks; conduct regular clinical reviews and audits; liaise with GPs, pharmacists and allied health professionals to coordinate care; collaborate with care managers and other Registered Nurses to support complex clients; delegate and guide care staff based on clinical needs; support falls prevention and client safety planning; provide palliative care guidance when required; and offer coverage for another RN’s visits during leave; travel to client homes and work within a hybrid (in-home) arrangement.
Required Qualifications
- Current AHPRA registration as a Registered Nurse (Division 1)
- Experience in aged care, community care, disability or related sector
- Current driver’s licence, reliable vehicle and comprehensive car insurance
- Willingness to travel to client homes across the Gold Coast
- Current Federal Police Check (or willingness to obtain)
