Job Summary
Provides in-home, community-based and telehealth mental health treatment to adults 55+ with a diagnosed mental health condition. Delivers psychoeducation, advocacy, and care coordination; supports development of annual treatment goals; maintains client communications with families and professionals; ensures timely documentation and compliance with regulatory and funding requirements; may perform assessments for licensed staff and facilitate Day Treatment Groups; requires strong communication, documentation, and organizational skills, with the ability to work across housing and community resources to support clients.
Required Qualifications
- Bachelor’s degree in social work or related field
- Current Ohio driver’s license and access to personal transportation
- Proficiency in computer programs (Microsoft Outlook and Office) and electronic health records
- Demonstrated strong planning and organizational skills
- Attention to detail and ability to meet deadlines
Desired Qualifications
- Bachelor’s degree in social work or related field
- Current Ohio licensure as an LSW preferred
- Three years’ experience in social service field preferred
- Excellent verbal, written, and electronic communication skills
- Proficiency with Microsoft Outlook and Office; Experience with electronic health records
