Job Summary
Conduct psychosocial assessments to identify client needs, risks, and support systems while creating care plans that help clients meet their goals. Assist families in navigating care decisions, transitions, and long-term planning by identifying barriers and developing solutions. Build and maintain relationships with referral sources including hospitals, discharge planners, skilled nursing facilities, physicians, and community organizations to drive private-duty care placements. Educate partners on EverTrue Anywhere Care services and facilitate warm hand-offs between referral partners and intake teams. Track referral activity, outcomes, and pipeline progress daily, sharing insights with leadership while participating in weekly sales and marketing meetings.
Required Qualifications
- Bachelor's degree in Social Work (BSW)
- 5+ years of prior healthcare community liaison experience
- demonstrated success in referral and business development in home care or senior services
- Excellent speaking, writing, and sales skills
- Strong interpersonal, presentation, and relationship-building skills
- Self-motivation to work independently and as a team member
- Advanced knowledge of computer technology
- Database experience
- Ability to interact effectively with referral sources, EverTrue team members, residents, clients, families, visitors, and the public
- Ability to implement and interpret programs, goals, objectives, policies, and procedures
- Must possess and demonstrate skills and ability to effectively present information to management and public groups
- general knowledge of medical terminology
