Job Summary
Community Outreach Worker responsible for conducting community outreach, engagement, and retention support for individuals experiencing substance use disorders. Duties include developing and maintaining referral relationships, educating clients on treatment options (including MAT), utilizing motivational interviewing and trauma-informed approaches, coordinating referrals and services across internal teams and community providers, and maintaining HIPAA/42 CFR Part 2 compliant documentation. Supports underserved populations (including individuals involved with the criminal justice system and those experiencing homelessness) and participates in meetings with local, regional, and state partners to raise awareness of available services and improve treatment engagement and recovery outcomes.
Required Qualifications
- Minimum of Associate degree in Human Services, Social Work, Psychology, Health Sciences with background in substance use disorders
- Valid driver’s license and auto insurance
- Proficiency in Windows OS and Microsoft Office (Word, Excel, Outlook, PowerPoint)
- Experience with electronic medical/health record systems and documentation practices
- Ability to communicate effectively in verbal and written formats with clients and stakeholders
- Knowledge of treatment options including MAT and recovery supports
- Ability to engage diverse and underserved populations with trauma-informed approaches
- Coordinate referrals and follow-up with internal teams and community providers
- Maintain HIPAA and 42 CFR Part 2 compliance in documentation
