Community Outreach Worker (COW)

Job Summary
Conduct community outreach in Holmes, Madison, Leflore, and surrounding areas to address healthcare access needs for consumers and providers. Review reports to identify care gaps for screenings and utilize population health systems to contact patients for annual wellness visits, transition of care, and high-risk follow-ups. Collaborate with medical providers, schools, faith communities, and social service organizations to build relationships and remove barriers to health and social services. Inform and assist homeless individuals regarding access to health, dental, mental health, and basic needs while maintaining strict medical record confidentiality. Establish resource lists, develop educational materials, and attend community meetings to facilitate consumer access to healthcare services.
Required Qualifications
- Bachelor's degree or 100 college hours in a related field, e.g., sociology, social work, special education, psychology, guidance, and counseling
- 2 years' experience and/or training in a health care setting
- Knowledge of the Mallory CHC's community culture and understanding of the community's socio-economic makeup
- Occasional travel to other clinic sites and as necessary for optimum performance of duties and responsibilities
