Job Summary
Support patients in navigating and accessing services, collaborate with care team to promote care plan understanding, assist with scheduling and home visits as directed, conduct eligibility determination and enrollment for uninsured patients, facilitate outreach and health screenings, provide referrals to community services, document and report in accordance with policies, and assist patients in securing transportation to appointments.
Required Qualifications
- A valid driver’s license and reliable transportation is required
- One of the following licensures from the Maryland State Board of Professional Counselors and Therapist: A valid Alcohol & Drug Trainee (ADT) or Certified Alcohol and Drug Counselor (CADC)
- Basic Microsoft Word and Excel experience
