Job Summary
Community Outreach Specialist—Harvey: Support patients on FGC’s mobile medication unit in Cook County/Harvey by conducting initial assessments, arranging transportation, and ensuring access to sustained treatment. Deliver medications on the mobile unit to patients enrolled in treatment; conduct outreach to people who use drugs and those experiencing homelessness; oversee GPRA data collection for mobile van services; document services in patient records per IL.DASA and FGC standards; describe programs and eligibility in layman’s terms to help individuals access treatment; coordinate daily with hospital detox staff, MAR outpatient centers, and intake staff to broker transportation and coordinate referrals. Requires personal lived experience with substance use and recovery; high school diploma or equivalent; valid driver’s license; commitment to confidentiality. Preference for knowledge of MAT, hospital-based detox processes, and CRSS certification.
Required Qualifications
- High School Diploma or equivalent
- Lived experience with substance use and stable recovery
- Familiarity with federal and state confidentiality laws
- Certification/CRSS preferred
- Possess and maintain a valid driver’s license and current vehicle insurance
- DCFS clearance required
