Job Summary
Coordinate client case management and implement individual plans of care for residents in independent housing programs. Provide transportation for day-to-day appointments and deliver training on independent living skills to support overall mental health stability. Serve as a role model, mentor, and advocate to recovering individuals, while assessing current and prospective needs through personalized supervision and goal setting. Requires ability to work evenings, weekends, and holidays; valid driver's license and reliable transportation with state-mandated insurance coverage. Successful completion of a criminal history background check is required after hire.
Required Qualifications
- High school Diploma or GED
- Ability to work alternative work schedules, evenings, weekends, and holidays as needed for operations and at the request of management
- Flexibility, dependability, and ability to work independently
- Valid driver's license
- Reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont
- Successful completion of a criminal history background check after hire

- Bachelor's Degree in Psychology, Counseling, Social Work or similar field
