Job Summary
Community Outreach Specialist responsibilities include promoting home care services in Westchester County, educating prospective clients and partners about eligibility and enrollment, developing targeted outreach plans, building relationships with healthcare professionals and community organizations, traveling within the assigned region to meet clients and referral sources, organizing community events and seminars, leveraging online channels for outreach, and tracking campaign performance. Requirements include a Bachelor’s degree, 2 years of community engagement/home care experience, knowledge of entitlements and insurance verification, and a willingness to travel within Westchester County. Required Qualifications
- Bachelor’s Degree required
- Experience with enrollment/intake for clients in a home/community-based setting
- Demonstrated knowledge of entitlements; Medicaid, DSS, DOH rules and regulations/insurance verification processes
- Experience with educating clients on health insurance benefits, entitlements, and assisting with the application/enrollment process
- History of developing and maintaining relationships with community partners and referral sources
- Access to personal transportation and willingness to travel within the assigned territory
