Job Summary
Community Outreach Representative responsible for driving business growth by developing and maintaining relationships with referral sources, healthcare providers, and community partners to increase patient census and promote agency services. Responsibilities include developing and executing strategic marketing and sales plans to grow referral volume, building relationships with physicians, hospitals, SNFs, case managers, and community organizations, conducting regular outreach through in-person visits, presentations, and networking events, identifying new business opportunities, educating referral sources on services and eligibility, collaborating with intake/clinical/operations teams to ensure smooth onboarding and service delivery, maintaining accurate CRM records, and staying informed on industry trends and market dynamics.
Required Qualifications
- Bachelor’s degree in healthcare administration, Business, Marketing, or related field (or equivalent experience)
- 2+ years of experience in healthcare sales, home care, hospice, or related field preferred
- Strong network within the healthcare community (physicians, discharge planners, etc.) is highly desirable
- Proven track record of meeting or exceeding sales targets
- Excellent communication, presentation, and relationship-building skills
- Self-motivated with strong organizational and time management abilities
- Knowledge of Medicaid, and managed care plans preferred
