Community Outreach Representative – Home Care

Community Outreach Representative to promote and grow Elite Home Health Care’s presence in NYC and surrounding areas. Build, maintain, and strengthen relationships with hospitals, nursing homes, physician offices, rehabilitation facilities, adult day health programs, social workers, case managers, and community organizations. Develop and manage referral sources to drive census growth and increase community awareness of agency services. Promote agency services through community outreach efforts, presentations, networking events, support groups, health fairs, and community-based events. Conduct home visits, intake support, and personalized appointments to assist with enrollment. Follow up promptly on referrals and leads from families, caregivers, healthcare providers, and community partners. Maintain ongoing communication with referral sources to ensure strong partnerships and exceptional customer service. Submit daily activity and outreach reports to management. Represent the agency professionally within the community and identify opportunities for market growth. Requirements include minimum 1 year of relevant experience; MLTC/Medicaid knowledge preferred; strong communication and interpersonal skills; ability to work independently; must have reliable transportation and ability to travel daily; bilingual Spanish/English preferred. Work Environment: Field-based / on-the-road position; Full-time, 8-hour shift. Salary range: $55,000–$60,000 annually, plus commission.
