Job Summary
Conduct prevention-focused outreach for individuals threatened with homelessness across East Hampshire, providing person-centered support to secure and sustain suitable accommodation. Carry out needs and risk assessments, support applicants to complete actions within their Personal Housing Plans, and help individuals access necessary services. Manage a caseload while working closely with partner agencies, landlords, and existing officers to build positive relationships. Maintain accurate records and work independently with flexibility within a 9 to 5 schedule, including occasional early mornings. Requires a full UK driving licence and access to a vehicle.
Required Qualifications
