Job Summary
Lead community outreach, partnership development, and field operations to expand NYREACH's footprint across New York City. Build relationships with health plans, provider groups, and local stakeholders to drive consistent referral volume and member engagement. Directly oversee the NYREACH Navigator field team, managing daily deployment, scheduling, and territory assignments while conducting on-site coaching to ensure compliance and performance targets. Track the full funnel from outreach to service delivery, identifying conversion gaps and implementing strategies to improve outcomes. Collaborate with clinical and marketing teams to align outreach efforts with available services and campaign execution.
Required Qualifications
- Bachelor's degree
- 3–6+ years of experience in community outreach, healthcare, non-profit, or related field
- Experience managing teams, preferably outreach or navigation staff
- Proven ability to build partnerships and drive program growth
- Excellent business development and relationship management
- Strong leadership and team development skills
- Strong organizational and operational execution skills
- Ability to manage performance metrics and improve outcomes
- Problem-solving mindset with attention to detail
- Adaptable and comfortable working both in the field and in-office
Desired Qualifications
- Master's degree
- Experience working with Medicaid and managed care
- Familiarity with Social Determinants of Health (SDOH) and Social Care Network (SCN) programs
- Experience overseeing navigator field teams or care coordination teams
- Bilingual (Spanish)
