Community Outreach Coordinator Level 1

Manage a geographic territory by mapping referral channels and cultivating relationships with 15 to 25 external organizations, including housing sites, criminal justice agencies, and physicians. Achieve monthly visit targets of 40 across primary care, dental, mental health, and HIV/AIDS services through effective prospecting and relationship building. Act as a liaison between external referral sources and internal programs, assisting clients with linkage to intake and assessment services while organizing health home intake assessments. Track all outreach activities in Salesforce CRM and develop annual account plans to support corporate goals. Provide excellent customer service, resolve complaints, and conduct educational sessions at community partner sites, with evening and weekend work available.
