Job Summary
Community Outreach Coordinator will conduct outreach to Medicaid-eligible clients with substance use disorders and related behavioral health needs, enroll them in services, coordinate with on-site nursing for metabolic screenings, arrange transportation to medical appointments, and maintain accurate records. Responsibilities include building trust with clients, collaborating with healthcare professionals, educating clients on health and wellness, and referring clients to appropriate resources. Requires a related-bachelor’s degree (preferred) or equivalent experience, proven outreach experience, a valid driver’s license, and strong communication skills; ability to work with diverse populations and participate in ongoing training.
Required Qualifications
- Bachelor's degree in social work, psychology, public health or related field preferred (relevant experience considered)
- Proven experience in community outreach, client engagement, or social services
- Knowledge of metabolic screening procedures and ability to record and interpret health data
- Valid driver's license
- Ability to communicate effectively and work with diverse populations
- Commitment to ongoing education and professional development
- Ability to work independently and in team settings
- Proficiency with computers and data-entry software
