Job Summary
Conduct home visits to assess client needs, design intervention programs, and evaluate home modifications ranging from simple to complex. Assist clients in selecting assistive technology solutions and maintain relationships with support coordinators, local area coordinators, and equipment suppliers. Document client progress and outcomes by writing detailed reports. This part-time or full-time role requires a minimum of three days per week and is situated in Melbourne's eastern suburbs. Candidates must hold an AHPRA-registered Occupational Therapy degree, a reliable car, and current NDISWC/WWCC. Required Qualifications
- A Degree in Occupational Therapy
- current AHPRA registration
- Experience or a keen interest in providing OT services to adults with disabilities, chronic disease, and complex care needs
- Strong time management and organisational skills
- Ability to complete autonomous home and community-based assessments and interventions
- A reliable car
