Job Summary
Provide comprehensive nursing care to palliative patients in their homes within the Halton area, including IV therapy, medication administration, and wound care. Organize daily priorities based on client health needs and develop, implement, and modify plans of care to achieve desired outcomes. Assess client health status and ensure electronic records are synchronized with the network twice daily. Attend team and POD meetings as required. This established part-time role involves 3 shifts per week and 1.5 weekends, with a bi-weekly requirement of 45 hours. Pay ranges from $34.76 to $37.64 per visit or $35.90 to $40.47 per hour. Candidates must have current CNO registration, 1–2 years of community nursing experience, and a valid driver's license.
Required Qualifications
- Current registration with the CNO
- Current CPR certificate
- Valid driver's license and access to a vehicle
- Must be legally eligible to work in Canada
Desired Qualifications
- Minimum 1 to 2 years clinical practice in a community environment preferred
- Minimum 1 to 2 years of nursing experience preferably in palliative care, hospice, or home health
- Certification in Palliative or hospice care (preferred)
