Job Summary
Develop and maintain effective care plans for patients in-home, clinics, retirement homes, or schools, providing exceptional nursing services through both in-person and virtual interactions. Liaise with patients, family members, and the interdisciplinary care team while performing other clinical duties as required. Work within your full scope of practice, utilizing a valid driver's license for regular travel to Stratford and surrounding areas. Rotate through 8 AM to 8 PM shifts over two weeks, inclusive of weekends and on-call work.
Required Qualifications
- Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
- Current College of Nurses of Ontario (CNO) registration in good standing
- A valid driver's license and vehicle
- CPR Certification
- Current immunization record including 2 step TB test
