Job Summary
Evaluate patients and collaborate with hospital personnel to determine clinical needs and admission appropriateness for skilled nursing facilities. Check benefits and payor sources while working closely with communities and healthcare partners. Requires a current Indiana LPN license and minimum three years of clinical experience in acute or long-term care. Schedule includes Saturday and Sunday shifts from 8a to 5p; remote work requires living within reasonable driving distance of Indianapolis.
