Job Summary
Overnight Community Night Care Practitioner role providing compassionate care in patients’ homes or remotely as part of our virtual ward. Work independently to support the virtual ward and urgent community pathways, with clear clinical support and escalation routes. Enable families to rest by ensuring loved ones are safe, and contribute to hospital avoidance through community rapid response. Training and development available; flexible night shifts including weekends and bank holidays. Prior experience in care is essential; palliative care experience welcome but not required. Location-based in Wolverhampton, West Midlands, UK with remote care delivery as part of the role.
