Job Summary
Community Navigator responsible for assessing client needs, providing referrals and intake screening for long-term services for older adults and individuals with disabilities in a call center and community-based settings. Duties include assisting with navigating application and enrollment for MI Bridges, Medicaid and ACA Marketplace plans, conducting assessments in diverse settings (hospitals, nursing homes, healthcare centers, schools, faith-based and non-profits), follow-up and case management, advocacy, crisis intervention, information and enrollment services, HIPAA compliance, and collaboration with internal teams and external partners. Position is in-person at the office with potential for a hybrid model after training and performance criteria are met.
Required Qualifications
- Experience providing information and referrals, community navigation, crisis intervention, advocacy, or case management
- Experience Medicaid, healthcare enrollment, public assistance, or benefits navigation programs
- Proficiency in Microsoft Office Suite and experience using call management systems, databases, and electronic documentation platforms
- Reliable transportation required
- Valid Michigan driver’s license and current auto insurance required
- Ability to travel throughout the service area based on organizational and client needs
- Obtain and maintain required certifications within designated timeframes, including Inform USA CIRS-A/D Certification, CMS CAC, MDHHS MI Bridges Training, SHIP Certification
