Job Summary
Oversee the clinical "Community Team" to manage patient health and wellbeing while delivering primary, urgent, and acute care to an assigned geriatric population. Collaborate with the Market GM and Senior Medical Officer to achieve quality outcomes, prioritize opportunities to improve community metrics, and foster key relationships with hospitals, SNFs, and community physicians. Act as a WellBe Ambassador to build contractual alignments and develop cross-functional partnerships that enable the clinical model, while floating within the community physician role as needed. Requires an MD or DO with 5+ years managing interdisciplinary teams and 5+ years in geriatric care, plus 3+ years managing a P&L. Full-time permanent role with daytime hours, flexible schedule, and on-call rotation; travel up to 100% locally required.
Required Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO)
- Current and valid state medical license
- 5+ years' experience managing an interdisciplinary team
- 5+ years' minimum experience as a community physician providing geriatric care
- 3+ years' responsibility for managing a P&L
- Valid driver's license
- Access to a car
- Willingness to drive to patient homes/patient location
- Active auto insurance policy
- Ability to lift up to 20lbs
- Moving lifting or transferring of patients may involve lifting of up to 50lbs
- Assist with weights of more than 100lbs
- Ability to stand for extended periods
- Fine motor skills
- Visual acuity
- Full cognitive and physical functioning
- Drug and alcohol testing compliance
- No marijuana use (including medical or recreational)
