Community Living Support Professional

Job Summary
Provide direct care and support to adults with developmental and intellectual disabilities in community settings. Assess clients' skills and interests, implement individualized care plans, document services in EVV/OASIS, assist with daily living activities (personal hygiene, meal preparation, medication administration, transportation), foster safety and inclusion, promote independence and socialization, collaborate with families and interdisciplinary teams, accompany participants to community activities as needed, and participate in ongoing trainings and staff meetings to maintain high-quality care. Requires teamwork, commitment to client well-being, and adherence to agency policies.
Required Qualifications
- Must be 18 years of age
- Must have a high school diploma or its equivalent
- Must have at least one year of experience working with adults with developmental and/or intellectual disabilities
- Must complete all required training as outlined by DDID regulations, including first aid and CPR certification
- Must be able to read, understand and implement written and oral instructions
- Must pass a criminal background check
- Must pass pre-employment and random drug testing
- Must have a valid driver's license, good driving record, and proof of auto insurance
- Must be available to work weekends and holidays
