Job Summary
The Community Liaison will be responsible for the preparation and smooth transition of new patients into the organization, visiting 6-10 facilities daily, building relationships with referral sources, and developing the community home health program. Key responsibilities include community outreach, coordinating admissions, delivering health educational services, and ensuring effective communication with home health staff and stakeholders. Applicants should have a minimum of 2 years experience in healthcare sales, a Bachelor's degree preferred, and possess strong organizational and interpersonal skills.
