Job Summary

Build and support relationships assisting persons living in homelessness to access healthcare services and resources. Facilitate linkage to community partners and coordinate follow-up for high-cost users while maintaining on-site presence at key agencies. Establish collaborative relationships with county government, safety net programs, and healthcare partners to address system gaps and support encampment outreach. Provide intensive accompaniment, consult on resource barriers, and analyze community capacity for service needs. Ensure timely documentation, develop community-level data for evaluation, and participate in key coalitions as a subject matter expert for Alliance staff and providers.

Required Qualifications

  • Bachelor's degree in a Human Services related field from an accredited college/university
  • Three (3) years of experience working with homeless populations and individuals facing complex challenges
  • Three (3) years of progressive experience in the field of Health Care or Human Services
  • Previous Wake County Homeless system experience
  • Permanent resident in Wake County or surrounding counties
  • Knowledge of the Homeless Management Information System (HMIS) and Continuum of care (CoC)
  • Ability to establish and maintain favorable working relationships with individuals, community stakeholders and government employees
  • Ability to build rapport and trust with homeless individuals
  • Demonstrated exceptional interpersonal skills, highly effective communication ability, and the propensity to make prompt independent decisions based upon relevant facts
  • Ability to make prompt independent decisions based upon relevant facts and demonstrated high level of diplomacy and discretion to effectively negotiate and resolve issues
  • Demonstrated knowledge of the primary purpose and functions of community agencies
  • Employment for this position is contingent upon a satisfactory background and MVR (Motor Vehicle Registration) check

Desired Qualifications

  • Must be available to report onsite to the Alliance Office for business meetings as needed
  • Required to travel weekly throughout Wake County community 2-4 days per week to meet with members and providers
