Community Liaison for Hospice

Build and maintain effective relationships with key referral sources and community partners to advocate for Medicare benefits and hospice services. Develop and coordinate the referral process to ensure seamless transitions for patients and families, while presenting program requirements to healthcare and community audiences. Identify areas to increase referrals and implement marketing plans that align with BJC Home Care's mission to serve thousands of patients across Missouri and Illinois. Maintain positive connections with customers and describe program requirements to increase awareness of available services. Full-time 40-hour schedule with some weekend coverage required.
