Job Summary
Assess community educational needs regarding hospice care and services on an ongoing basis. Conduct chart audits to ensure compliance and monitor perceptions of Bristol Hospice as a high-quality provider. Serve as a liaison between facilities, physicians, and community organizations to coordinate referrals and admissions. Respond promptly to customer and referral source requests, complete consent forms, and submit documentation per administrative processes. Provide patient and family education on hospice philosophy and care while collaborating with clinical supervisors to schedule intake activities. Maintain a comprehensive working knowledge of community needs and applicable information systems.
Required Qualifications
- Active LVN license in the state of California
- Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order
- Must be willing to submit to a criminal background check as well as a drug/alcohol screening upon hire and periodically thereafter
- Must demonstrate vital sensory abilities including near vision, far vision, perceptual speed, visual color discrimination, depth perception and hearing sensitivity
- Must have finger dexterity, arm-hand steadiness, and manual dexterity
- Must be able to communicate with patients, families, and other health care workers
- Must be able to perform prolonged or considerable walking or standing
- Must be able to lift supplies and equipment
- Must be able to perform considerable reaching, stooping, bending, kneeling, and/or crouching
- Must have visual acuity and hearing
- Must be flexible in work hours
- Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
- Must be compassionate without becoming personally involved with the patients, and their families
- Must be dependable and flexible in work schedule
- Must be able to lift and carry up to 50 lbs. with or without assistance
- Must be able to spend a prolonged amount of time standing, walking, bending, and stretching
- Must possess the ability to deal with stressful situations
- Must have excellent observation, verbal and written communication skills, problem solving skills
Desired Qualifications
- One to two years of experience in Hospice Sales preferred
