Job Summary
Community Liaison role in Americare Senior Living focusing on connecting the skilled nursing facility with the broader healthcare community. Serve as the primary contact for referral sources, families, and discharge planners; coordinate tours and admissions; build relationships with hospitals, physicians, and community partners; represent the community at networking events and outreach activities; perform follow-up with patients after discharge; travel up to 50% with a full-time schedule. Requires a high school diploma or GED (LPN/RN/BSW/MSW or therapy background preferred) and strong communication/interpersonal skills. Driving is required.
Required Qualifications
- High school diploma or GED
- Valid driver’s license with good driving record
- Ability to travel up to 50%
- Education/Experience: LPN, RN, BSW, MSW, or therapy background preferred; case management/admissions/skill nursing experience preferred
