Job Summary
Community Liaison to support the Adult Day Health program by growing referral networks, guiding admissions and enrollment, and expanding community awareness. Responsibilities include developing and maintaining referral relationships with hospitals, physician offices, SNFs, social workers, elder services, and other partners; conducting in-person outreach visits; representing the organization at events; delivering educational sessions; managing admissions from inquiry to enrollment; coordinating intake activities and facility tours; and collaborating with clinical/operational teams to ensure smooth onboarding. The role also involves marketing support, open houses, and tracking referrals and outcomes, with frequent local travel within the service area. Requires strong interpersonal and organizational skills, ability to work independently, proficiency with CRM and Microsoft Office, and a valid driver’s license; bilingual candidates are encouraged to apply.
Required Qualifications
- Bachelor's degree or equivalent combination of education and experience
- Minimum 1–3 years of experience in healthcare outreach, community relations, admissions, business development, marketing, or related fields
- Valid driver's license and reliable transportation
- Proficiency with Microsoft Office and CRM/referral tracking systems
- Ability to conduct presentations, tours, and community outreach activities
Desired Qualifications

- Existing relationships within local healthcare, senior services, or community services networks desirable
