Job Summary
Conduct assessments of individuals' background and circumstances to determine service eligibility, then develop plans addressing needs in single or multiple areas. Network with community agencies to coordinate resources and support systems, while initiating and attending individual and group meetings in community-based settings. Provide transportation for individuals and facilitate skill development groups, ensuring all services align with established plans. Submit documentation regarding services provided and evaluate individual progress against measurable goals. This role is funded by The Behavioral Health Grant and serves clients associated with the Adult Residential Center to reduce barriers to community reintegration.
Required Qualifications
- Bachelor's degree in Human Services or related field
- May require KS/MO driver's license
- Johnson County Government requires reference/background screening
