Job Summary
Part-Time Community Integration Counselors provide individualized services to support individuals with disabilities in their community integration efforts. Responsibilities include encouraging independence, working with case managers on tailored programs, documenting progress, assisting with medical needs, driving clients to appointments, and ensuring accurate program documentation. A high school diploma or GED and a valid driver's license are required, along with at least 90 days of relevant experience. The position includes physical activities and may require on-call participation.
Required Qualifications
- High School Diploma or GED
- Valid driver’s license with a satisfactory driving record
- Minimum 90 days of relevant experience supporting individuals with intellectual and developmental disabilities
- Ability to establish a comfortable and supportive relationship with individuals receiving supports
- Ability to exercise good judgment and remain calm in crisis situations
Additional Requirements
- Frequent lifting, stretching, and other physical exertion may be required
- May be required to lift or move 25+ pounds
- May be exposed to various medical conditions and communicable diseases
