Job Summary
The Community Integration Counselor role involves assisting individuals with learning life skills through individualized services aimed at community integration. Responsibilities include supporting clients' independence, collaborating with Case Managers to tailor programs, documenting progress, and coordinating transport to activities and medical appointments. Qualifications include a High School Diploma or GED, a valid driver’s license, and at least 2 years of experience in support for individuals with disabilities. Paid training is provided, and the position includes opportunities for professional growth.
Required Qualifications
- High School Diploma or GED
- Valid driver’s license with a satisfactory driving record
- Minimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilities
Desired Qualifications
- Bachelor’s Degree preferred
- Excellent verbal and written communication skills
- Excellent time management skills
- Ability to establish a comfortable and supportive relationship with individuals receiving supports
- Ability to exercise good judgment and remain calm in crisis situations
Additional Requirements
- Must cooperate with the licensee and department staff in any inquiry, inspection or investigation
