Job Summary
Support and encourage individuals to increase their ability to be as independent as possible in physical, intellectual, emotional, and social life skills. Meet with waiver participants regularly in residences, the community, or office settings to provide ongoing support for community integration efforts. Document individual progress, administer medications, and ensure proper completion of necessary documentation. Coordinate leisure and volunteer opportunities while monitoring health and medical needs, immediately reporting concerns. Drive individuals to medical appointments and community activities. Ensure program documentation and billable records are completed accurately and timely. Cooperate with license and department staff in any inspection, inquiry, or investigation.
Required Qualifications
- High school diploma or GED
- Valid driver's license with a satisfactory driving record
- Minimum of two years of relevant experience supporting individuals with intellectual and developmental disabilities
- Ability to establish a comfortable and supportive relationship with individuals receiving supports
- Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing
Desired Qualifications
