Job Summary
The Community Integration Counselor will support individuals with disabilities by assisting them in enhancing their independence and community integration. Key responsibilities include developing and facilitating programs in collaboration with case managers, monitoring health and medical needs, documenting progress, providing ongoing support, and coordinating volunteer opportunities. A minimum of two years of relevant experience, a high school diploma or GED, and a valid driver’s license are required. The role demands excellent communication and time management skills.
Required Qualifications
- High School Diploma or GED
- Valid driver’s license with a satisfactory driving record
- Minimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilities
Desired Qualifications
- Bachelor’s Degree preferred
- Excellent verbal and written communication skills
- Excellent time management skills
- Ability to establish a comfortable and supportive relationship with individuals receiving supports
- Ability to exercise good judgment and remain calm in crisis situations
