Community Integrated Case Manager

Job Summary
The Community Integrated Case Manager provides case management services in shelter or housing programs and acts as the liaison between the CBI Shelter and Housing Programs and the CBI PCMH clinic. Responsibilities include assisting the patient and treatment team in planning, connecting, outreaching, advocating, coordinating, and monitoring the patient’s needs and progress. The role involves assessing patient needs, developing and monitoring individualized treatment plans, and documenting services in an accurate and timely manner. A minimum of six months of recovery from substance use and/or mental health disorders is preferred, along with relevant education and work experience in behavioral health.
Required Qualifications
- High school diploma or GED
- Behavioral Health Technician (BHT) certification in accordance with A.A.C. R9-10-101 and CBI Clinical Policies and Procedures
- Current AZ Driver's License (valid and in good standing)
- Clear 39-month Motor Vehicle Record
- Arizona Fingerprint Clearance card (Program specific)
