Community Healthcare Worker - PRN - (Centralia/Marion County)

Conduct field-based home visits to assess social, environmental, and basic health needs, assisting patients in navigating healthcare systems and community services. Provide patient check-ins via phone or virtual platforms, advocate for access to services, and support scheduling of appointments, care follow-ups, and referrals. Deliver education on chronic disease prevention, wellness, nutrition, and medication safety while connecting members to housing, food security, and behavioral health resources. Maintain accurate documentation in the Electronic Medical Record, track patient interactions, and collaborate with interdisciplinary teams on case reviews and quality improvement. Respond to staffing requests at least three out of every four times contacted, reporting to assigned shifts within two hours of unplanned absences. Maintain availability for a minimum of one eight-hour shift per week and four shifts per month to remain active in the PRN Staffing Program.
