Job Summary
Community Healthcare Specialist focused on driving referral business within senior housing and health care divisions. Responsible for patient education, support, coaching, and ensuring services are in place for patient success. Builds and expands relationships with physicians, hospitals, SNFs, senior agencies, and other senior communities; delivers educational presentations and materials, supports groups, conducts needs assessments, and performs advanced care planning for senior living communities. Primarily based in senior living communities with potential regional travel. Demonstrates company culture and values, promotes CSIG service lines, and coordinates with the community team to connect with medical professionals. Strong emphasis on presentation/public speaking, cross-functional collaboration, and delivering patient-centric outcomes.
Required Qualifications
- 1-3 years health care experience preferred but not required
- Social work or clinical liaison background preferred but not required
- Strong Microsoft Office Suite skills (Word, Excel, PowerPoint, TEAMS, Outlook)
- Strong presentation and public speaking skills required
- Good communicator with ability to articulate product lines and services
- Sales skills
- Solutions-oriented
- Ability to manage multiple projects independently
- Ability to multitask and prioritize in a fast-paced environment with attention to detail
- Good judgment and sensitivity to confidential and non-routine matters
