Job Summary
Community Health Worker to service patients in the Bluffton/Okatie area, performing home and ALF visits, delivering medications and DME, coordinating telehealth visits, and supporting care teams to ensure access to care and coordinated, compassionate service. Responsibilities include facilitating visits with PT/OT/providers, performing medication reconciliation, enabling remote patient monitoring and RPM training, delivering DME, collecting venipuncture specimens as needed, and training patients/caregivers in proper use of provided equipment. Must have reliable transportation and be able to travel between offices, facilities, and patient homes. Qualifications emphasize CMA/paramedic/nurse licensure preferred, phlebotomy experience or willingness to train, and comfort with technology platforms and EMRs. Full-time role with 8:00 AM–5:00 PM shifts in the Bluffton/Okatie service area.
Required Qualifications
- Certified Medical Assistant, Paramedic, or Licensed Nurse preferred
- Phlebotomy experience preferred
- Previous experience in public health or home health preferred
- Experience with DME delivery or logistics operations preferred
- Must be proficient in using technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records
- Strong organizational and time management skills
- Ability to read and communicate effectively
- Must be able to work independently with minimal supervision
- Must be a licensed driver with automobile in good working condition
- Must have a clean driving record
