Job Summary
Conduct home and facility visits to coordinate patient care, deliver medications and durable medical equipment, and perform routine venipuncture for laboratory testing. Facilitate telehealth visits with specialists and therapists, instruct patients on remote monitoring equipment, and ensure medication reconciliation accuracy. Communicate effectively with care teams and providers via phone and electronic platforms to maintain compliance with care plans. Requires reliable transportation for daily travel within the Columbia service area, working 8-hour shifts from 7:00 AM to 4:00 PM.

- Proficient in using technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records
- Strong organizational and time management skills

- Ability to read and communicate effectively
- Strong written and verbal communication skills
- Basic computer knowledge
- Ability to work independently with minimal supervision and direction
- Good interpersonal and communication skills
- Ability to work flexible hours
- Must be a licensed driver
- Automobile insured in accordance with state and/or organization requirements
- Automobile in good working condition
- Clean driving record
- Experience operating vehicles in a professional capacity
Desired Qualifications
- Certified Medical Assistant
- Paramedic
- Licensed Nurse
- Certifications or licenses in good standing
- Phlebotomy experience
- Previous experience in public health or home health
- Experience with DME delivery or logistics operations
