Job Summary
Community Health Worker (PRN, hourly) servicing the Charleston area with visits to patients' homes and Assisted Living Facilities, delivering medications and durable medical equipment, coordinating telehealth visits, and working with care teams to ensure coordinated patient care. Requires reliable transportation and ability to travel daily; shifts run from 7:00 AM to 5:00 PM in designated service areas such as North Charleston/Moncks Corner. Preferred certifications include Medical Assistant, Paramedic, or Nursing licenses, with phlebotomy and public health/home health experience encouraged. Responsibilities include fostering caregiver and patient education, medication reconciliation, RPM equipment setup/training, and handling DME delivery and specimen collection as needed.
