Job Summary
Community Health Worker responsible for linking patients to healthcare services, performing home and facility visits (ALFs/ILFs), delivering medications and DME, coordinating telehealth between patients and care teams, training on and supporting remote patient monitoring, and ensuring coordinated, compassionate care across South Carolina service areas. Must have reliable transportation; full-time role with 12-hour shifts (7:00AM-7:00PM) in North Augusta, Warrenville, and Gloverville. Must hold relevant certifications or licenses (medical assisting, paramedic, or nursing) and be able to work with care teams to improve access to care, medication reconciliation, RPM support, and DME delivery.
Required Qualifications
- Certified Medical Assistant, Paramedic, or Licensed Nurse preferred
- Phlebotomy experience preferred (or willingness to train)
- Experience in public health or home health preferred
- Experience with DME delivery or logistics preferred
- Proficient with electronic medical records and telemedicine tools
- Must be licensed driver with insured vehicle in good standing
- Reliable transportation required for daily travel
- Excellent communication and organizational skills
- Ability to work independently and across teams
- Ability to travel between offices/facilities as needed
