Job Summary
Community Health Worker (CHW) to service patients in the Bluffton/Okatie area, performing home and ALF/ILF visits to facilitate care coordination, deliver medications and DME, support telehealth/remote monitoring, and assist with specimen collection and collaboration with care teams; requires reliable transportation and full-time schedule (8:00 AM-5:00 PM) in a physician-group setting.
Required Qualifications
- Certified Medical Assistant, Paramedic, or Licensed Nurse preferred
- Phlebotomy experience preferred or willingness to training in phlebotomy
- Experience in public health or home health preferred
- Experience with DME delivery or logistics operations preferred
- Proficient with technology tools for remote patient monitoring, telemedicine, EMRs
